  • Tower's Watson OneExchange (formerly Extend Health) The HEWT is partnering with Tower's Watson OneExchange to provide Medicare-eligible participants a variety of individualized healthcare plans. Tower's Watson OneExchange Open Enrollment elections are the same dates as Medicare open enrollment. You may visit the Tower's Watson OneExchange website at https://medicare.oneexchange.com/hewt. Or you may call them at 1-888-864-0764 (TTY:  1-866-508-5123).  The hours are 9:00 a.m. to 9:00 p.m. Pacific Time.

    This includes the tables and the instructions for filling out the W-4P.  PLEASE READ THE INSTRUCTIONS PRIOR TO FILLING OUT THE FORM!
  • ACH Direct Payment   (PDF)
    Tired of writing a check every month for your insurance premiums? You can now have your Medical and Life Insurance premiums automatically deducted from your checking or savings account. To enroll, please complete this form and return it to us.
  • Beneficiary Designation Form (PDF)

Pension Plans
  • 2013 Annual Lump Sum Factors (PDF)
    These factors are based on the August 2012 PPA segment rates and change annually.  In order for the 2013 rates to apply, distribution forms must be submitted to MSA Benefits prior to December 1, 2013.
  • 2014 Annual Lump Sum Tables for 10 Year Certain (PDF)
    These factors apply to the portion of your benefit earned prior to 1/1/2014 as a 10 Year Certain.  These factors are based on the August 2013 PPA rates and change annually.  In order for the 2014 rates to apply, distribution forms must be submitted to MSA Benefits prior to December 1, 2014.
  • 2014 Annual Lump Sum Tables for Straight Life Annuity (PDF)
    These factors apply to the portion of your benefit earned after 12/31/2013 as a Life Annuity.  These factors are based on the August 2013 PPA segment rates and change annually.  In order for the 2014 rates to apply, distribution forms must be submitted to MSA Benefits prior to December 1, 2014.
