 Portable Benefit Plans

• Employees may apply for a portable Long-Term Care insurance through Seabury and Smith.  You may request the information packet and application from Benefits Administration.  Questions should be directed to Seabury and Smith at 1-800-932-1132.

• Group Universal Life Insurance is a portable plan available through Marsh.  You may request an information packet and application from Benefits Administration.  Questions should be directed to Marsh at 1-800-642-5726.
